A specialty that covers large and diverse fields

Some of the common procedures performed in this field includes removal of wisdom teeth, treatment of tooth root cysts, dental apicoectomies (which involve cutting off the root tip of an infected tooth when recommended by the endodontist) and soft and hard tissue biopsies.

It is also closely related to periodontics and dental implants. On many occasions, due to bone and gum loss, the conditions are not ideal for the placement of an artificial denture. Therefore, it is necessary to perform pre-prosthetic treatments on soft and hard tissues before, during or after implant placement. This guarantees an adequate foundation for the long-term success of the dental prosthesis.

Oral medicine and surgery: corrective and preventive solution

Oral surgery also deals with injuries that may arise in the oral mucosa, gums or bone. They can appear in the form of white, red, yellow or mixed spots, ranging from canker sores or papules to ulcers, have an irregular appearance and varied symptoms.

Many of these lesions are of viral or bacterial origin or are caused by fungi, some extrinsic factors (cocaine, snus, chewing tobacco, alcohol…) and even by trauma, producing lesions that could evolve malignantly. Likewise, there are also oral lesions that manifest as part of a set of signs and symptoms of a syndrome.

It is important to note that early detection of oral cancer is essential to prevent this disease. Many types of cancer first manifest in the mouth and later metastasize to other parts of the body.

Preprosthetic surgery

Pre-prosthetic surgery procedures allow the removal of excess soft tissue and bone, and also regenerate them, in case there is a lack thereof. This type of surgery can be performed for the placement of dental implants or for the placement of removable prostheses. The goal is to achieve good bone and soft tissue support, so that the prosthesis can better fit and support.

Vestibuloplasty

This surgery is performed to obtain a greater fundus of the vestibule. We achieve this by increasing the distance between the correct position of the teeth or dental implants and the insertion of the non-keratinized gum in the buccal bone that is in contact with the lips.

In a patient without teeth, the alveolar bone is lost. This causes a resorption of the bone ridge and the removable prostheses, since they do not have a clamping area, move.

In patients with teeth, this surgery is performed when the vestibular frenulum has an insertion very close to the teeth. The gum is migrating downward, giving the impression of a longer tooth. In these cases, a frenectomy or a vestibuloplasty is usually performed, depending on the type of gingival recession that the tooth has.

Elimination of malformations and bone growths

Sometimes bone overgrowths occur, both in the maxilla and in the mandible, which may be torus or exostosis. These protuberances can increase in size to the point of preventing the correct seating of a prosthesis, without pain or mobility. When they occur on the tongue, if they grow too large, they can affect phonation and chewing.
Why if they are benign can they hurt? Because on the protrusion there is a minimal layer of gum, which causes pain when placing the prosthesis. The patient will feel a kind of pinch in the gum, especially during chewing.

Through oral surgery we remove or polish the existing protuberances, with the aim that the prosthesis can be adjusted correctly and without pain.

Autologous tooth transplant

Oral dental transplant surgery consists of changing the position of a tooth, from its own socket to a different socket. It is performed in aesthetic treatments, for example, when placing a retained tooth in the position in which it should be. In this way, through this procedure, we managed to recover the functionality and aesthetics of the teeth.

It is also quite common to transplant wisdom teeth to another location where the patient is missing a tooth and the bone and soft tissues are adequate. In some cases, after a few months it is necessary to perform a root canal treatment and an onlay inlay, to ensure the best possible occlusion and that the patient can chew comfortably.

Closure of an oro-sinus communication

Oro-sinus or oro-nasal communication can occur when a tooth is extracted and there is no bony barrier separating the mouth from the maxillary sinus, nasal fossa or pteregoid space. In this case, the patient feels that when he drinks or speaks there is a communication between his nose and his mouth. This fact can also occur after orthognathic surgery or as a complication of dental implant treatment.

The closure of this communication, sometimes, is done spontaneously, but in others not, being necessary the closure to avoid possible sinusitis and infections. Nowadays, techniques are used, such as coronally displaced flaps with or without connective tissue graft, or rotary palate flaps with or without Bichat ball displacement.

Displacement of the inferior alveolar nerve

As we age, both the alveolar bone and the soft tissues of the oral cavity that support the teeth are lost. Once the teeth have already fallen, this process becomes more noticeable. In some cases, this becomes so atrophic that it causes the inferior alveolar nerve, as it exits through the mental foramen, to be very close to the bone ridge, where the prosthesis rests. This causes that, when the patient places the prosthesis on this area, they experience pain and decide not to use their removable prosthesis anymore.

Through the transposition and lateralization of the inferior dental nerve, we can achieve the placement of dental implants, which will allow the prosthesis to be implant-retained and the patient not to feel any pain when chewing.
